Division of Quality Assurance Northeastern Regional Office Move

The purpose of this memo is to inform providers of the relocation of Bureau of Health Services (BHS), Bureau of Nursing Home Resident Care (BNHRC), Bureau of Assisted Living (BAL), and Bureau of Licensing, Technology, Education (BTLE) personnel currently housed at 200 North Jefferson St., Suite 211, Green Bay.

The Division of Quality Assurance Bureaus will be moving from 200 North Jefferson St., Suite 211, to 1325 South Broadway, DePere on March 19, 2009.
